Reconnections

This page gives an overview of reconnections policies, and the key considerations that frontline outreach staff need to make when trying to reconnect clients to former locations.

Aims of reconnections policies

  • To encourage rough sleepers back to families and recognised positive networks.
  • To encourage new arrivals to return, in a planned way, to an area where accommodation and support is available.
  • To offer a client centred assertive resettlement process for rough sleepers.
  • To encourage cross-authority working.

Key considerations for frontline staff

It is important to establish if the person wishes to reconnect back to where they have lived. Some rough sleepers have moved away to escape negative lifestyles or networks and reconnection would be a backward step. Rough sleepers who refuse to reconnect to an area can not be prevented from accessing Supporting People services on the grounds that they have no local connection in an area in which they currently reside. Therefore, support should be available whether the rough sleeper wishes to reconnect or not.

The networks of support available to the person at each location may be a factor in deciding if reconnection is suitable. It can be valuable to outline the services available in the present location and any barriers that the client may face when trying to access them.

Returning from reconnection may be an issue; effective throughcare and partnership working with cross-authority agencies (including contact with final destinations) can be effective in lowering return rates.

Contact with local partnership agencies can be valuable in ensuring a smoother reconnections transition. Mental health services and specialist support such as Barka can give longer term sustained support to reduce the chance of relapse or return.

Has an assessment of the individual’s support, health and housing needs been undertaken?

Does the person wish to return to another area? Do you have consent for this?

Is it safe for the person to return to another area?

Has the receiving authority/housing provider been notified to ensure accommodation and/or other support services are available for the client in the receiving authority.

Have arrangements to facilitate the reconnection been agreed?

Have travel costs and arrangements been completed?

Is someone able to accompany the person to the train/bus station so they can make their journey to the receiving authority?

Has the receiving authority been advised that the client is en route?

Has there been a follow-up contact that the client has arrived safely?
